Stepping Stones offers comprehensive case management services to assist residents in identifying and accessing area resources to address daily living, medical, mental health, psychiatric, substance abuse, family planning and domestic violence issues.  All residents are assigned a Case Manager to advocate on their behalf.  Each resident will meet with their Case Manager and other Stepping Stones staff a minimum of 1x weekly for a Small Team Meeting.  The Case Manager will assist in establishing appropriate community services and making referrals as needed.  Additionally, a resident’s Case Manager will maintain contact with DHHS and/or the placing agency to report progress and identify resident strengths and needs.  The Case Manager, along with the Clinical Supervisor, will continue to assess the needs and goals of each resident.  Each resident participates in creating an individual treatment plan to guide services based on an individual resident’s strengths and needs.   

 

GROUPS

 

Stepping Stones offers a variety of therapeutic groups for children and their biological parent(s) taking part in the residential program.  All residents are encouraged to participate in parenting and nurturing education.  Parenting/nurturing education occurs regularly as part of daily living at the Stepping Stones residence with support staff as well as part of mandatory Life Skills, Parent to Nurture and Nurturing Parenting groups.  Residents are encouraged to attend the different educational groups offered throughout their stay.  These groups may include Dress for Success, Health and Safety, Stress and Fitness, Budgeting, Stress Management, Taking Charge, Expressive Art, Meal Preparation and Nutrition.

THERAPY

 

Stepping Stones offers diagnostic and individual therapy services for residents on site with a licensed clinician.  All residents are required to attend individual and/or group counseling as outlined in their Individual Treatment Plans.  Resident may also seek therapy services elsewhere in the community or continue with their current therapist if located in the immediate area.  Case Management will assist residents for referrals for all services needed, as well as other pertinent groups in the area.

 

CHILD DEVELOPMENT COORDINATION

 

All residents will encouraged to participate in 1:1 and group services with a Child Development Coordinator to identify and address resident strengths and needs related to age appropriate development, family roles and expectations. 

 

COMMUNITY INTEGRATION

 

Stepping Stones community integrations services are designed to encourage and assist  parents in locating and accessing resources available to their family in their current location as well as their area of origin.  Community Integration works 1:1 with parents to strengthen and build independent living skills, as well as maintaining a household through access to employment, volunteering and education.   

 

PUBLIC HEALTH NURSING

 

Stepping Stones ensures that all children are assessed by a Public Health Nurse once monthly to ensure proper growth and development.  Stepping Stones works regularly with local visiting nurse agencies and parenting support services.  Stepping Stones invites the Public Health Nurse into the residence to provide services.
