Stepping Stones offers comprehensive case
management services to assist residents in identifying and accessing
area resources to address daily living, medical, mental health,
psychiatric, substance abuse, family planning and domestic violence
issues. All residents
are assigned a Case Manager to advocate on their behalf.
Each resident will meet with their Case Manager and other
Stepping Stones staff a minimum of 1x weekly for a Small Team
Meeting. The Case
Manager will assist in establishing appropriate community services
and making referrals as needed.
Additionally, a resident’s Case Manager will maintain contact
with DHHS and/or the placing agency to report progress and identify
resident strengths and needs.
The Case Manager, along with the Clinical Supervisor, will
continue to assess the needs and goals of each resident.
Each resident participates
in creating an individual treatment plan to guide services based on
an individual resident’s strengths and needs.
